If illusions, spell or item click, are going to be in-game, will it be a real illusion that affects NPC response (i.e. faction), actual player characteristics (size, etc.), underwater breath (if a fish) or just cosmetic for appearance only?
This assumes that race affects faction and that animal forms might have a bias also.
I remember in EQ having a Druid turning me (a Troll) into a wolf so that I could turn in belts in Freeport to gain guard faction. Which in itself was kinda strange that guards wouldn't mind big black wolves running around in the city.

Just a few threads from a quick search where this topic has been discussed.
Illusions are not "real" by definition. They are tricks of the mind meant to cloud sensory perception. It changes what the eyes see. You cannot see underwater breathing. Illusions do not change the nature of the person the illusion is cast upon. Transformation/Transmogrification or some such changes the actual physiology of the target.
Illusions would have an effect of faction, say if an Ogre walked up to a Human, but he saw the Ogre as another human.
Looking like a fish should not grant underwater breathing. If you transform into a fish, you shouldn't be able to carry a sword or cast magic. You're now a fish.
